Leave of Absence


Taking a Leave of Absence

Life happens - both good and bad - and sometimes that means you may need to take a leave of absence from your UC Health job. Regardless of the reason for your leave, here are the steps you need to take to ensure UCLA Health Sciences knows your intent and that you continue to be covered by any benefits for which you are eligible.

Step 1

Inform your department head, manager or supervisor about taking a leave of absence as soon as possible.

Please notify your department head, supervisor or manager of your leave request 30 days prior to your anticipated leave begin date, or as soon as reasonably possible. It's best to do this as soon as your need for a leave arises, so your department can plan your absence. Try to be as specific as possible about the length of time and reasons for the leave of absence.

Once you are assigned to a LOA Analyst you can setup a one-on-one consultation appointment or to schedule a phone conference to discuss the details of your leave.

Step 4

Complete your Leave of Absence Packet.

Print and complete the employee section in the Leave of Absence Packet and have the respective physician complete the Health Care Provider Certification (This form should be completed and returned within 15 calendar days, if unable to do so please contact your LOA Analyst). This packet should be completed when you meet with your LOA Analyst.

Step 5

Submit your Leave of Absence Notification (LOAN).

Complete and submit your Leave of Absence Notification (LOAN) and follow as instructed. Your respective leave may be taken on an intermittent or reduced schedule basis. Please ensure that your LOAN is approved by your supervisor/manager and forwarded to your LOA Analyst.

Step 6

Make decisions about which benefits you'll continue during your leave.

Make sure you understand how going on leave affects your UC-Sponsored health and welfare benefits, as well as your service credit in the UC Retirement Plan. For many benefits, including medical and dental, you have the option to continue them for up to two years, so long as you pay the premium (both you and UC's portions).

To review the benefits, you're enrolled in, sign in to your account and choose "Current Enrollments" under the health and welfare option.

If you have an outstanding 403(b) Plan loan, you'll need to suspend or set up monthly payments during your leave, or repay the loan in full. Contact Fidelity Retirement Services for more information.

If you have parking you will need to make arrangements with Parking Services to continue or cancel your parking.

Step 7

Stay in Touch.

Be sure to keep your address and other contact information up to date while you're on leave. Keep your supervisor/manager informed of any changes or delays on your return date. If there is any need to extend your leave, you must inform your supervisor/manager and LOA Analyst immediately by submitting a doctor's note that states your new anticipated return to work date.

Step 8

When you return, contact your local benefit office.

They can inform you about what you need to do to get back to work. Within 31 days of your return to work, you'll need to restart benefits you chose not to continue during your leave. If your leave was less than 120 days, you may re-enroll yourself (and your eligible family members) in the same plans, with the same coverage levels as before. After a longer leave, you may enroll in any UC-sponsored plans which you're eligible.

Step 9

Returning to Work.

Submit your Return to Work Certification form to your supervisor/manager and LOA Analyst prior to your first day back from work. If you cancelled your benefits when you were on leave, you have 31 days from your return date to work to re-enroll by submitting a UPAY-850 to your Benefits Analyst.

Step 10

If your UCRP service credit was suspended.

If you are not getting paid while on leave, you usually don't earn UCRP service credit. You may be able to buy back any service credit for your leave period. Generally the sooner you do this, the less it will cost you.

Step 11

If you do not return to work at UC.

Check with your local benefits office for information about continuing your group insurance plans.

If you decide to retire from UC, contact the Retirement Administration Service Center.
